Tina Butler, LPC is an in-network provider for the following insurance companies:

• Aetna
• Cigna Healthcare and Cigna Behavioral Health
• United Behavioral Health

Late Policy - In order to keep from inconveniencing other patients, if you are more than 15 minutes late for your appointment, we will ask you to reschedule the appointment.

No-Show Policy - There is a 24-hour cancellation policy. Cancellations with less than 24 hours notice will incur a charge at insurance payment rate, which will be billed to the Responsible Party, as it is not billable to insurance.

Co-pays and Uncovered Services - We expect payment at the time of service for all co-pays and uncovered services.   In the event that your account goes to collection, there will be a 20% collection fee added to your balance.

Refills - Office Policy requires 72 hours notice for prescription refills.

Cell Phones - We ask that you refrain from cell phone use in the office.

Office is located on the second floor of the building. No elevator. No Handicapped Access.

Pre-authorizations - It is the patients responsibility to obtain pre-authorization before the first visit if your insurance policy requires that.
